Home Health Quality Improvement National Campaign Agency of the Month
HHQI will randomly select an Agency of the Month (AOTM) demonstrating "Examples of Excellence" from all completed entries submitted by the 25th of each month. To submit your entry, please complete the following form and click submit.
The winning agency will be notified by e-mail. As AOTM, the agency will be honored by being awarded a prominent logo indicating this distinction and their testimonial will be placed on the HHQI Web site and in the e-bulletin.
Using the blank spaces below, please tell us the story of how your agency is improving the quality of care your home health patients receive. While describing, consider what changes your agency has made to improve the overall quality of care you deliver to patients.

Disclaimer: Participants closest to the 90th percentile or higher in ACH & Oral Medication Management rates demonstrating active participation in the 2010-2011 HHQI National Campaign are eligible for random selection as HHQI AOTM. 90th percentile = 16.22% for ACH and 66.7% for Oral Medication Management. To see your current rates, log-in to HHQI Data Access. If your agency is below the threshold right now, please check your HHQI Data Access reports monthly to see your current rate. Reports are updated around the 20th of each month.
