If you are selected as our Agency of the Month, your organization’s logo will be prominently featured here; on the front page of this website; and in HHQI Inside Edition, our monthly e-newsletter that reaches almost 14,000 subscribers. Your achievement will also be shared via HHQI's social media channels and archived on our list of past honorees.

Be in the spotlight.

Would you like the chance to show the nation how committed to quality improvement your home health agency is? Then you’ve come to the right place. The Home Health Quality Improvement (HHQI) National Campaign will randomly select a home health Agency of the Month (AOTM) each month demonstrating Examples of Excellence from all completed entries. Please see the disclaimer below for more information on eligibility requirements.

The agency selected as AOTM will be notified by e-mail and awarded certificate of recognition indicating this distinction. Additionally, the agency's name, logo, website link, and a short quote will be featured on the HHQI website and in the HHQI Inside Edition e-newsletter.

Wouldn’t your supervisor be thrilled to see your agency on a national stage? Of course he or she would. So, let’s get started now. Using the form below, please tell us how your agency is improving the quality of care your home health patients receive. While describing, consider what changes your agency has made to improve the overall quality of care you deliver to patients.

All fields must be completed to be eligible for the HHQI Agency of the Month drawing but you only need to complete this once.  Your entry will be “rolled over” to the following months if your agency is not selected. Disclaimer: Participants closest to the National Risk Adjusted ACH 20th Percentile and / or the National Risk Adjusted Oral Medication Improvement 80th percentile (or better)  demonstrating active participation in the current HHQI National Campaign are eligible for random selection as HHQI AOTM.  To see your current rates, log in to HHQI Data Access.  If your agency is currently below the threshold, please monitor your HHQI Data  reports monthly to confirm eligibility. Reports are updated around the 23rd of each month.
